The Ontario Provincial Police reported the crash on Monday at around 8:30 p.m. on McDowell Road East at Charlotteville Quarter Line in Norfolk County.
Police said two people were taken to hospital. One of the two suffered "life-altering" injuries, while police have not released the severity of the other person's injuries. The driver of the van was uninjured.
Police said charges are pending.
People in southern Huron and Lambton counties are being told to limit their time outside.
