Police, Warwick Fire and Rescue and Lambton EMS responded to Forest Road and London Line just before 5 p.m. September 20.
One driver, 57-year-old Robert Hollingsworth of Warwick Township, was pronounced dead at the scene.
The other driver was taken to hospital with non life-threatening injuries.
Ahlyssa Van Es has been charged with careless driving causing death.
