Hospital-to-Home program expands throughout Windsor-Essex

A partnership that helps patients transition from hospital care to home recuperation faster is expanding throughout the region.

Hospital-to-Home (H2H) is building on a successful launch at Windsor Regional Hospital by expanding to Hôtel-Dieu Grace Healthcare (HDGH) and Erie Shores Healthcare (ESHC).

The partnership with St. Elizabeth Health provides up to 16 weeks of guidance to patients who no longer need acute care in hospital, allowing them to recuperate in the comfort and privacy of their homes.

"Partnerships like H2H demonstrate the value of regional collaboration in strengthening care close to home," said ESHC Interim President and CEO Katelyn Dryden. "By working together with Windsor Regional Hospital and our regional partners, we are able to connect patients with enhanced supports following discharge, helping them recover safely at home while improving the continuity of care across the region."

HDGH President Bill Marra added that the program does much to support patients once the discharge papers are issued.

"Having a coordinated approach to care for our patients is a vital piece in ensuring positive patient and family outcomes. This collaboration will further our goals of helping patients transition safely back to their home with the resources and support they require," said Marra.

The H2H program was launched at Windsor Regional Hospital in October 2025. Since then, 303 patients have participated.

In February, the provincial government announced an almost $2-million dollar investment to expand H2H, part of a $1.1-billion commitment to boost home health care in Ontario.
