Discharge patients get informed through new app

Patients discharged from Chatham-Kent hospitals can now have all of their follow-up questions answered through an app.

The Chatham-Kent Health Alliance (CKHA) has launched a new tool in an effort to ease the discharging process. An app called CoHealth (formerly known as Dash MD) launched within the CKHA earlier this month.

CoHealth will allow patients to access information in terms of post-op instructions, follow-up appointments, and medication reminders. As stated in a media release sent by the CKHA, the app also allows patients to find nearby health care providers for follow-up care.

"Being discharged from the hospital can be a stressful and overwhelming experience for patients and their families," said Lisa Northcott, vice president and chief nursing executive. "By empowering patients and families to manage their care after discharge we help ensure that our patients have a successful recovery, without complications or readmission."

This is one of the tools the CKHA has recently unveiled for patients. A paper-based tool called Patient Oriented Discharge Summaries, launched earlier this year.

The CoHealth app is available for iOS and Android devices.
